Screening Process
What Happens During a Screening?
A screening is a short telephone call or face-to-face meeting between you, as a potential client and a care coordinator. Your unique situation and needs are discussed as are services offered by the agency. If you are eligible for services, an intake appointment is scheduled to meet with your care coordinator.
You will be asked to bring the following to the intake:
Proof of income
Proof of HIV/AIDS status
Photo ID (preferably Oklahoma)
Proof of residency, if ID is outside of Oklahoma state.
If you lack any of or all of these documents, the care coordinator can offer advice on how to obtain them during your screening.
