Testing, treatment, care, cure.

Our program offers person-centered care coordination and linkage to high-quality medical care for persons living with Hepatitis C. The program uses cutting edge care coordination, person-centered action plans and referrals, and direct support to assist individuals who have had difficulty accessing treatment.

Hepatitis C Classes and Intake

We believe in personal empowerment through education, agency, and peer support. Therefore, the HCV program conducts bi-monthly educational classes and group intakes to ensure successful treatment. The HCV classes are unique and have proved to be an strong resource for clients who need supportive services to access treatment.

Farmer’s Market and Weekly Client Meals

Participants are eligible for bi-weekly FREE fresh produce and weekly Second Cup lunch.

Transportation Assistance

Program clients can access transportation assistance to help get to medical and lab appointments.

Comprehensive Testing

Once our clients complete the HCV educational class, they are eligible for  testing which helps our partner agencies prescribe the best individualized medication regimen, which have a 98% cure rate[1].
