Care Coordination
A care coordinator is a professional who becomes a partner in helping you to attain your maximum health. Through strong working relationships, clients and their care coordinators map out ways to access health care and other social services that assist with basic living needs such as housing, food and clothing. Each client is assigned a primary care coordinator who partners with the client to meet their needs.
Care coordinators can help clients access:
Prescription assistance
Dental, vision and medical care
Mental health services, counseling and support groups
Housing, food and nutrition therapy
Medical transportation
Social Security, Medicaid, Medicare and private insurance.
All of Tulsa CARES’ care coordinators have either a bachelor’s or a master’s degree in social work or a related field. Tulsa CARES also has one bilingual care coordinator who can assist Spanish-speaking clients in navigating social services and medical-related appointments.
