Care Coordination

Tulsa CARES maintains a staff of professionals to provide care coordination services. All of our counselors have a bachelor’s degree and 60 percent have or plan to obtain a master’s degree in social work, or a related field. Tulsa CARES has one bilingual care coordinator who will interpret information for clients at medical-related appointments.

Eligibility – Under the Ryan White CARE Act, clients are eligible for care coordination services regardless of income. However, to be eligible for financial assistance under the Ryan White CARE Part B, clients must have an income that is less than 200 percent of the Federal Poverty Guideline. This guideline is revised each year by Congress, and typically increases with each update. Tulsa CARES serves residents living in any of the 23 counties in Northeastern Oklahoma.

Potential clients should contact Tulsa CARES to be screened for eligibility, information and referral. This allows a potential client to talk with a care coordinator to determine if the services offered through this agency can benefit that person. Individuals who are within the financial eligibility criteria of the Ryan White Part B services are offered the opportunity to become a client and are able to begin accessing Tulsa CARES services. Individuals who do not meet the eligibility criteria of the Ryan White CARE Act Part B services are offered information and referrals.
